Admission Requirements for Interested Students

First Phase Childrens Academy admits students of any race, color or ethnicity to all rights, privileges, programs and activities generally accorded or made available to students at the school. Each prospective applicant and his/her parents/guardian must do the following:

  • Arrange a tour and interview with the admissions office.
  • Submit a certified copy of the student’s birth certificate.
  • Submit a State of Florida Immunization Record and physical form.
  • Submit a copy of the student’s social security card.
  • Submit Award letter from scholarship (Step for Students, McKay or Gardiner)
  • Recent Report cards and Test scores
  • IEP if applicable
  • All previous school records
